The incumbent supports the Behavioral Health Department to include the Behavioral Health providers with support services such as but not limited to: Behavioral Health intakes and screenings, comprehensive community support services (CCSS), screening brief intervention and referral for treatment (SBIRT), group facilitation, and support services at the colleges and School Based Health Centers (SHBCs). This position is allocated 90% to direct care coordination and support services, 10% to direct patient services such as SBIRT/CCSS.
90% Care Coordination: The incumbent's primary role is to support the Behavioral Health department and other departments within ECFH with effective communication to include but not limited to answering phone and scheduling appointments. Assist with coordination of behaviora1 health referrals, warm hand-offs and maintain an active list of community resources, assist the behavioral health providers with primary screens (i.e., risk assessments, PHQ-9, GAD-7, ACE. PCL-5, social determinants of health) to identify client needs and to assist with the data entry such as the Treat First process.
10% SBIRT/CCSS: The incumbent's secondary role is to conduct SBIRT services, identifying individuals with substance use disorders and those at risk of developing these disorders. The incumbent process includes providers screening, brief intervention, and referrals for treatment when necessary. Brief Intervention can include psychoeducation, support services, crisis intervention, and skills building.

Job Description:
 
Provides compassionate, effective outpatient clinical wound care to patients, often in fast-paced environment. May case-manage a group of patients, will work under guidance of clinic physicians and Clinical Coordinator, provide patient/caregiver wound related education, communicate with other healthcare providers regarding a patient's care, enter and evaluate data, maintain patient charting.

Requirements:
 
<ul>
<li>Active RN license within the State of practice.</li>
<li>Bachelor's Degree in Nursing preferred.&nbsp;</li>
<li>BLS issued through the American Heart Association required, ACLS preferred.&nbsp;</li>
<li>Must possess a certification as a Wound Specialist or as a Certified Wound Care, Ostomy, and/or Continence Nurse.</li>
<li>Basic/operable computer skills.&nbsp;</li>
<li>Must have obvious concern for patient welfare, sound organizational skills, ability to analyze data, read, understand and implement professional journals as well as regulatory documents, ability to learn and use wound care products, multi-task and possess effective/mature communications skills.</li>
<li>Ability to read and speak Spanish a plus.</li>
<li>Experience in general nursing, wound care, hyperbaric oxygen and outpatient clinical operations preferred.</li>
<li>Exposure to infectious disease, bloodborne pathogens and caustic chemicals; occasional unpredictable physically demanding situations.</li>

Responsibilities
About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers: 
<ul>
<li>Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols</li>
<li>Administer and order point of care and lab-based testing as needed</li>
<li>Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate</li>
<li>Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary</li>
<li>Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities</li>

<li>Flexibility – Visits are scheduled based on YOUR availability. We conduct assessments Sunday – Saturday from 8 am – 7 pm. You can work during the day, evening, and/or weekend based on your schedule. </li>
<li>Competitive Pay – Earn income based on the number of assessments you complete. Increase your earning potential by offering greater availability and traveling to see members outside your home location. Those completing a monthly average of 20 – 100 visits earn $1,800 – $11,500 per month. This is based on the per assessment completion rate range of $90 – $115 depending on your state of residence. </li>

<li>Education and Support – We invest in YOU! In addition to comprehensive onboarding training we will assign you a preceptor, provide 1:1 feedback, and offer free access to earn continuing education. </li>
<li>Additional Licensure – Should business need exist; we will support you in obtaining additional state licensure and credentialing in neighboring states – or others you may want to visit while completing health assessments. </li>
<li>Full-time (FT) Opportunities – Our PRN providers can transition to FT roles supporting their home location, or a state or region – and receive a FT sign on bonus.</li>

Qualifications
Skills &amp; Experience That Shine
<ul>
<li>Master’s Degree or commensurate experience and satisfactory completion of NP licensure</li>
<li>Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed</li>
<li>AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty</li>
<li>Current BLS, ACLS or CPR certification</li>
<li>1 year of NP experience preferred, new grads encouraged to apply!</li>
<li>Valid state driver's license and proof of adequate automobile insurance coverage for the state of residence</li>
<li>Strong computer skills and familiarity with employee health/medical record software</li>
<li>Excellent verbal and written communication skills </li>
