PRODUCTIVITY
The Home Visiting Service Provider will achieve the expected 100% productivity performance goal set by MECA Management.

<li>The Home Visiting Service Provider will see each family assigned one time per month for a minimum of 90 minutes</li>

DOCUMENTATION
The Home Visiting Service Provider will accurately collect data on persons served and will ensure that the information is accurate and filed in a timely manner by adhering to the following:

<li>Documenting therapy sessions and progress
<ol>
<li>Documentation to occur at the time of visit and entered into CYFD database within 24 hours of visit.</li>
<li>Print all Data Assessment Plan (DAP) notes and place in childs file within 24 hours of visit</li>

STAFF DEVELOPMENT AND TRAINING
The Home Visiting Service Provider will provide the necessary support, training, and guidance in their provision of services to the families served. The Home Visiting Service Provider must also comply with all mandatory trainings identified by the Early Childhood Education and Care Department (ECECD) as well trainings mandated by their respective supervisor(s) and/or company by adhering to the following:

<li>Consulting and providing in-house trainings to other staff members</li>
<li>Obtaining Infant Mental Health (IMH) Certification within 2 years of hire</li>
<li>Attending mandatory staff meetings and trainings</li>

SUPERVISION

The FSC will be assigned a weekly caseload from the EC program director or designated staff member.

PRODUCTIVITY
The FSC will achieve the expected 100% performance goal of providing an average of two hours per month of service coordination for EACH child on their caseload as written in the childs IFSP.
 
DOCUMENTATION
The FSC will complete necessary data collection and reporting tools as mandated by MECA executive management, programs, and primary contracting agencies including NM Department of Health and Local Education Agencies and will ensure that the information is accurate and filed in a timely manner by adhering to the following:

<li>Create and maintain a confidential record for each person served which includes the following:</li>
<li>signed consent and release forms</li>
<li>current evaluation and assessment results</li>
<li>documentation of eligibility determination</li>

<li>Document all time spent with the family and work done on behalf of the family</li>

STAFF DEVELOPMENT AND TRAINING
